The NPCHD Immunization Program includes vaccines for both children and adults.

Childhood vaccines:

Appointments are necessary for childhood vaccines. Please call 330.364.4491 to schedule an appointment.

• Immunizations given to children ages 1 month to 18 years.
• Parent or guardian should bring immunization records if available.
• $15.00 for initiial vaccination.
• $10.00 for each additional vaccination (each child during same visit).
• $15.00 for flu shot.

Adult vaccines:

Adult vaccines are available on a walk-in basis, Monday-Friday 8:30-11:30, 1:30-4:00.

NPCHD will bill insurance when possible, otherwise $20.00 for each vaccine.

• Td/Tdap
• Varicella (Chickenpox)
• Zostavax (Shingles) - Available with insurance only

• Covid vaccines (call for availability) 
• $30 for Flu Shot
• Pneumonia
